This simple form acts as your team registration for the upcoming season. The Intent to Race Form needs to filled out by every team and submitted no later than the date listed on the form.

Certified Roster [32K – Microsoft Word]
The Certified Roster form is the only way that a school and inform the league that a student plans on racing. All racers are entered into our league program from this list so it is important that it be turned in, in alphabetical order. Once this list is turned in there can be no more additions to your team. If your team has cuts then a second form is turned in that lists the racers that were cut, not the remaining participants.

Race Recording Form [19K – Microsoft Excel]
The Race Recording Form is used to submit official race results.

Team Invoice [31K – Microsoft Word]
This form is to be submitted to your Athletic Director or School Administrator so the CISL can be compensated for your participation in the league. Please be as accurate as possible so the numbers listed on this form and equal to those listed on your Certified Roster. Please remember that this payment is due the first two week of January.

Final Team Invoice [31K – Microsoft Word]
This form is to be used only if you initial payment was too much. This happens when your payment is sent based on the numbers on your Certified Roster form is too high because of team cuts. Refunds are not given because of academic ineligibility or loss of interest. This form can be submitted when you realize there is a numbers discrepancy.
